When we have “PHP Dispatching” checked, the resulting file is not readable. De-selecting this, result in the image just being opened in the current window.
So the URL being exposed results in not actually downloading file, such much as redirecting to it, and setting the feature to obscure the URL trashes the file being served.
What are we missing here?
The following tutorial will explain why the image opens in the browser by default and what you can do to force a download of it:

Regarding the PHP Dispatching, what kind of file is it? How big is the file? Is it hosted on your site (or is it coming from an external site)?
