I’ve created the download item several times according to the video on the Squeeze Form page, followed by new page. But when I click the Select Download Item field, nothing displays. What’s wrong?

Can you please send us a screenshot or link us to the page where this issue is occurring so we can get a better understanding.
Do you mean once you’ve set it up you do a trial you cannot successfully download the item?

Here’s the link to a page I created that displays 4 screen shots: 2 for the Download page, and 2 for the page that displays the download:

Please let me know what you find out.
Thanks so much!
March 7, 2019 at 5:51 am #1031adminKeymasterWe will investigate this and apply a fix. Please copy and paste the shortcode in your post/page in the meantime.
Below is the shortcode that you can use. Replace 123 with the actual download ID of your site:
[sdm-squeeze-form id="123" fancy="0" button_text="Download Now"]
March 9, 2019 at 3:07 pm #1032adminKeymasterThis has now been fixed. Please upgrade to the new version.
April 9, 2019 at 4:58 am #1051BillBurns87ParticipantI downloaded the Download Monitor plug-in, then bought the Email Lock, installed them both, created a PDF download and a page in WordPress. It wasn’t working, so I contacted you and was given this code to use in the meantime: [sdm-squeeze-form id=”123″ fancy=”0″ button_text=”Download Now”] Now the contact form displays on my page. But when I submit a name and valid email into the form, I get an error page that says: “This page isn’t working. [my URL] is currently unable to handle this request. HTTP ERROR 500.” I went back into the Admin page on my site and clicked the All Downloads icon to make sure my selected download was OK. However, I now get an error page that says: “500 Server Error A misconfiguration on the server caused a hiccup. Check the server logs, fix the problem, then try again. I don’t know where or how to check the server logs or fix the problem. But I can’t access my downloads at all. Finally, regarding the first issue with them Download Monitor and Email Lock, you recently sent me this message: “This has now been fixed. Please upgrade to the new version. Post Link: https://simple-download-monitor.com/forums/topic/squeeze-form-download-item-not-showing-in-select-field/#post-1032” I don’t know if that upgrade instruction refers to the free Download Monitor or the purchased Email Lock. I also can’t find a way to upgrade either of them. Please help asap! I’m trying to post my latest site and can’t do it with these issues unresolved. Thanks a lot!
April 10, 2019 at 4:44 am #1054adminKeymasterFor the 500 error, you need to ask your hosting provider to look at the server log and give you the details. If you give us the actual error from the server log, I will be able to provide more help.
What changes did you make to the site when you started to see this error?
The update was for the main simple download monitor plugin. If your plugin version is v3.7.4.2 or higher then you are fine:
